Welcome to our Dental Membership Program
Welcome to our annual dental savings program designed for families and individuals seeking quality dental care at reduced prices. Our program at Benicia Family Dentistry offers immediate benefits and savings, without the limitations of deductibles, yearly maximums, or waiting periods commonly found in traditional insurance plans. We are excited to offer this exclusive plan to you and your family, providing access to exceptional dental care at our office.
Our Benefits/Savings include:
- 2 regular healthy teeth cleanings per membership period (valueof $23)
- 2 complete dental exams per membership period (value of $146)
- All x-rays needed for annual exams (value up to $152)
- 1 Velscope oral cancer screening per membership period (value of $20)
- 2 fluoride treatments per membership period (value of $90)
- A 20% savings on general restorative dental procedures
- A 15% savings on all major restorations
- A 10% savings on items from our store
ANNUAL MEMBERSHIP FEES:
- Primary member: $425.00 (a savings of over $20)
- Additional family members: $375.00 each (a savings of over $250)
ELIGIBILITY:
Eligible family members include spouses and dependent children under the age of 19 (up to age 26 if the dependent child is a full-time student). All members on the membership family account will have the same anniversary date as the primary member, and the membership period is for one year from the registration date.
PAYMENT TERMS:
- Fees are due and payable at the time of service and are non-refundable once services have been provided.
- All patient portions for services received are due at the time of services to receive Membership savings.
- Payment plans of 6 to 60 months duration are available upon request with approved credit. Repayment duration is based on service totals and procedural type. Selected payment plans may result in a reduction in your Membership savings.
APPOINTMENT RESCHEDULING:
Please notify our office at least 48 hours in advance if you need to change your reserved appointment.
